JOB DESCRIPTION
Collaborate with providers to individualize medication therapy to promote safe, effective, and cost‑conscious oncology care Perform comprehensive medication therapy management and support safe transitions of care Monitor and adjust drug regimens as referred, including ordering and interpreting laboratory tests
Provide refill authorization with assessment of adherence, efficacy, safety, and drug interactions
Serve as a drug information resource for providers, nursing staff, research staff, and patients
Ensure patient access to specialty and oral chemotherapy medications, including providing clinical documentation to insurers as required Identify opportunities to optimize therapy for under-treated oncology patients
Assess and address barriers to medication adherence, including cultural, physical, cognitive, emotional, and socioeconomic factors
Educate patients on disease self-management and oral chemotherapy regimens
Review clinic prescribing patterns and provide targeted education aligned with national and institutional oncology guidelines
Manage anticoagulation, infectious disease therapy, pain/palliative care, or complementary and alternative therapies under collaborative practice agreements, as applicable
REQUIRED SKILLS AND EXPERIENCE
Graduation from an accredited School of Pharmacy
Active Washington State Pharmacist license
PGY2 Oncology Pharmacy Residency OR a strong clinical oncology background
Demonstrated experience managing oral chemotherapy agents
Minimum of two years of pharmacy practice experience
ASHP-accredited residency training may substitute month‑for‑month for experience
