Job title: Licensed Clinical Social Worker
Job location: New York, NY 10016
Shift: 9:00 AM-5:00 PM
Length: 3 months, Possible Extension
Pay: $60 - $64/hr
Qualification/Requirements:
- 1 year LCSW experience.
- Valid Licensed Clinical of Social Work (LCSW) license issued by NYSED, Primary Source Verification required.
Additional Details:
- Along with multi-disciplinary team, engages 68 clients in community and office visits at least once weekly in order to provide ongoing wellness and medication management, assessment, and other supports as needed
- Provides direct services related to the treatment and recovery of people living with mental illness according to ACT guidelines
- Demonstrates competencies in harm reduction, trauma-informed care, stage-wise treatment, motivational interviewing, and other evidence-based practices
- Displays core competencies in designated areas of practice, including the ACT core operating principles, integrated physical health and behavioral health, vocational services, family psychoeducation, and wellness self-management
- Coordinates treatment and services with a variety of other teams and systems including inpatient units, primary and specialty care physicians, supportive housing programs, substance use rehabilitation programs, respite care, etc.
- Completes goal-oriented treatment planning (plan of care, psychosocial, safety plans, etc.)
