Job Title: Care Management Specialist

Location: Remote - Downers Grove, IL

Contract Duration : 6 Months – Contract to Hire

Work Hours: Monday to Friday | 8:00 AM – 5:00 PM

Summary:

Responsible for discharge care coordination, transition of care, and episodic case management for members with acute health conditions. Provides pre-admission and post-discharge support, patient education, medication reconciliation, appointment coordination, and referrals to community resources to improve health outcomes and reduce readmissions. Collaborates with providers, members, and care teams to address barriers to care, develop care plans, and navigate the healthcare system. Determines case complexity and refers members to appropriate programs when needed.

Requirements:

• Current, active, unrestricted LCPC or LMSW license.
• 2+ years of clinical experience.
• 1+ year of health insurance/managed care experience.
• Knowledge of medical management policies and procedures.
• Strong communication, customer service, and care coordination skills.
• PC/database proficiency.
• Ability to travel occasionally and possess a valid driver's license.
