Licensed Master Social Worker

We are seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to support patients in an inpatient setting. The LMSW will provide psychosocial assessments, supportive therapy, discharge planning, and referral coordination to help patients and families navigate healthcare challenges and ensure safe transitions of care. This role requires strong clinical judgment, effective communication skills, and the ability to collaborate with interdisciplinary healthcare teams.


Key Responsibilities

  • Conduct comprehensive psychosocial assessments to identify patient and family needs.
  • Provide supportive therapy and emotional support to patients and their families.
  • Develop and coordinate discharge plans to ensure continuity of care and safe patient transitions.
  • Facilitate referrals to community resources, rehabilitation programs, social services, and support agencies.
  • Collaborate with physicians, nurses, case managers, and other healthcare professionals to address psychosocial barriers to care.
  • Advocate for patients and families to ensure access to appropriate services and resources.
  • Document assessments, interventions, treatment plans, referrals, and patient progress accurately and timely.
  • Assist patients in navigating healthcare systems and accessing community-based support services.
  • Participate in interdisciplinary care planning and case management activities.
  • Maintain compliance with regulatory, ethical, and professional social work standards.


Qualifications

  • Master's Degree in Social Work (MSW) from an accredited program required.
  • Active Licensed Master Social Worker (LMSW) license issued by the New York State Education Department (NYSED) with Primary Source Verification required.
  • Minimum one (1) year of LMSW experience required.
  • Experience in inpatient medical-surgical, hospital, or acute care settings preferred.
  • Strong knowledge of psychosocial assessment, supportive therapy, discharge planning, and referral management.
  • Excellent communication, interpersonal, counseling, and advocacy skills.
  • Ability to work effectively within a multidisciplinary healthcare team.


Important Details:

  • 13-week contract position (extensions available)
  • Shift is 7-hour day (Monday–Friday, 9:00 AM – 12:00 PM), 35 hrs/week
  • Location is New York, NY 10016
  • Pay Range for Locals: $55 - $60 per hour
  • Pay Range for Travelers: $2,350 - $2,400 per week
