We are seeking an experienced Home Health Physical Therapist (PT) to provide skilled therapy services to patients in their homes throughout the Corpus Christi service area. The ideal candidate will have a strong clinical background in home health, be comfortable working independently in the field, and possess excellent assessment, documentation, and care coordination skills.
This role focuses on conducting Start of Care (SOC) visits, evaluations, reassessments, and discharge visits while developing individualized treatment plans that improve patient mobility, functional independence, and overall quality of life.Coverage Areas

Key Responsibilities- Perform comprehensive physical therapy evaluations in the home health setting.
- Conduct Start of Care (SOC), reassessment, recertification, and discharge visits.
- Develop and implement individualized treatment plans based on patient needs and physician orders.
- Assess mobility, strength, balance, gait, functional limitations, and rehabilitation potential.
- Deliver evidence-based therapeutic interventions to improve patient independence and safety.
- Educate patients and caregivers on exercise programs, fall prevention, mobility techniques, and use of adaptive equipment.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and interdisciplinary care teams.
- Complete timely and accurate clinical documentation, assessments, and treatment notes.
- Ensure compliance with Medicare, state, federal, and agency regulations.
- Meet productivity expectations while maintaining high standards of patient care.
Required Qualifications- Active Physical Therapist (PT) License
- Recent Home Health Physical Therapy experience
- Experience performing SOCs, evaluations, and discharge visits
- Strong clinical assessment and patient management skills
- Valid driver's license, reliable transportation, and auto insurance
- Ability to travel throughout assigned coverage areas
- Excellent communication and interpersonal skills
Preferred Qualifications- OASIS proficiency
- Homecare Homebase (HCHB) experience
- Strong time management and organizational skills
- Dependable and self-motivated professional
- Experience managing an independent home health caseload
Mileage Reimbursement- Mileage reimbursed between patient visits, from the first patient to the last patient of the day.
