PRN Contract Occupational Therapist (Home Health)

Location: Lafayette/Acadiana, LA and Surrounding Areas

We are seeking a compassionate and dedicated Contract Occupational Therapist (OT) to provide PRN services in a home health setting throughout Lafayette/Acadiana/New Iberia and surrounding communities. 10 to 20 Visits per week. This position is ideal for an Occupational Therapist who is passionate about helping patients regain independence, improve daily functioning, and achieve their personal goals in the comfort of their own homes.

Responsibilitie

  • sEvaluate patients and develop individualized treatment plans based on functional needs and goals
  • .Provide skilled occupational therapy interventions to improve activities of daily living (ADLs), instrumental activities of daily living (IADLs), mobility, safety, and independence
  • .Educate patients, family members, and caregivers on treatment techniques, adaptive equipment, home safety, and strategies to maximize independence
  • .Communicate and coordinate with physicians, home health agencies, and interdisciplinary team members to ensure continuity of care
  • .Document evaluations, treatment plans, progress notes, and discharge summaries in a timely and accurate manner
  • .Reassess patient progress and modify treatment plans as appropriate
  • .Recommend adaptive equipment, assistive devices, and environmental modifications to enhance patient safety and function
  • .Comply with all federal, state, and professional regulations governing occupational therapy practice
  • .Maintain a patient-centered approach while delivering high-quality care

Qualifications Requir

  • edCurrent and valid Occupational Therapist (OT) license in the State of Louisian
  • a.Graduate of an accredited Occupational Therapy progra
  • m.Current CPR and First Aid certificatio
  • n.Strong clinical assessment and treatment skill
  • s.Excellent communication, documentation, and interpersonal skill
  • s.Basic computer proficiency and electronic documentation experienc
  • e.Reliable transportation and valid driver's licens
