The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. The Incident Management Director (IMD) works under the direct supervision of the State Supported Living Center

(SSLC) Director. Performs managerial work administering the daily operations and activities of incident

management department of the SSLC. Establishes goals and objectives, and manages goal evaluation activities

for the department.� Ensures Center's compliance with facility goals and quality improvement initiatives, ICF-IID

Regulations, statutory requirements, SSLC policies and procedures and applicable laws related to incident

management. Identifies areas of needed change and makes recommendations for continuous quality

improvement. Establishes an administrative review process for discovered, non-serious injuries of unknown

cause, to reduce the rates of unusual incidents at the facility and proactively addresses potential issues. Tracks

and coordinates unusual incidents, ensures proper reporting and transmits information to appropriate personnel.

Assigns, prioritizes and manages the activities and professional development of the SSLC investigators, campus

administrators, administrative assistants, and other assigned staff. Ensures 24-hour coverage of incident

management functions including all weekends and holidays. Provides oversight and quality control of the

unusual incident investigative process and the work performed by investigators.� Serves as a subject matter

expert, provides technical expertise and guidance. Conducts unusual incident investigations in investigator's

absence and as assigned by SSLC Director. Facilitates critical incident meetings to review high profile incidents

to ensure adequate protective measures and corrective actions are implemented. Monitors all unusual incident

corrective action plans, prepares reports and monitors budget expenditures. Interacts and exchanges information

with various state and federal agencies, participates in workgroups and manages all activities related to the daily

Incident Management Review Team (IMRT) meeting. Manages the review of trend analysis data related to

unusual incidents. Responds to emergencies, coordinates the center's response to emergencies, and works with

local organizations related to disaster planning. Performs other duties as assigned. Works under general

supervision, with limited latitude for the use of initiative and independent judgment. Some travel may be required,

and work schedule may vary depending on services needed.

Essential Job Functions (EJFs):

Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned. May serve in an on-call status on a rotating schedule as required to meet operational needs of the facility.

Establishes goals and objectives for the incident management department based upon the strategic plans of the SSLC Director. Sets standards for achieving goals and manages goal evaluation activities by reviewing applicable data trends, reviewing and revised active plans of improvement, and providing routine updates to Center leadership. Ensures Center's compliance with facility goals and quality improvement initiatives, ICF-IID Regulations, statutory requirements, SSLC policies and procedures and applicable laws related to incident management. Identifies areas of needed change and makes recommendations for continuous quality improvement of incident management operations and processes to the SSLC governing body.�

Establishes an administrative review process for discovered, non-serious injuries of unknown cause, to reduce the rates of unusual incidents at the facility. Proactively addresses potential issues for the SSLC related to client safety which could include environmental modifications, additional staff training, modification to staff deployment, and client specific factors. Maintains and monitors tracking systems to identify systemic trends related to incidents including specific clients, incidents, alleged perpetrators, location and any other relevant factors.�

Administers the daily operations and activities of the incident management department. Tracks and coordinates unusual incidents including serious client injuries, and abuse/neglect allegations, and ensures incidents are reported to HHSC Complaint and Incident Intake (CII) and State Office within required time frames. Transmits information regarding incidents to relevant staff at the center to ensure appropriate protections are put in place. Ensures timely entry of data into the Client Abuse Neglect Reporting System (CANRS). Ensures systems are implemented to maintain security of confidential information and chain of custody of potential evidence.

Assigns, prioritizes and manages the activities of the SSLC investigators, campus administrators, administrative assistants, and other assigned staff based upon SSLC structure and needs. Hires, trains, and promotes professional growth of the employees within the department so that a competent workforce is maintained. Ensures that all departmental staff are compliant with training according to agency and facility standards. Manages the professional development of investigators and campus administrators including enhancing their skills in investigatory processes, root cause analysis, effective written and oral communication, and incident data analysis. Conducts performance evaluations of assigned staff to give employees timely feedback on their performance and oversees staff development plans and activities.� Takes action to improve employee retention and reduce turnover.

Provides oversight and quality control of the unusual incident investigative process and the work performed by investigators. Serves as a subject matter expert to the investigators, assisting them in organizing the evidence, determining and summarizing the facts, and drawing conclusions. Ensures timely implementation of preliminary investigations. Reviews and approves reports regarding allegations of abuse, neglect and exploitation developed by investigators to ensure professional quality and appearance, appropriate content, consistency, and responsiveness. Develops and implements techniques for evaluating the quality of work performed by investigators. Ensures timely completion of the final investigation report. Reviews evidence submitted for completion of recommendations to ensure appropriateness.� Conducts unusual incident investigations in investigator's absence and as assigned by SSLC Director.

Ensures organizational structure is set up for effective and efficient utilization of staff and available resources. Ensures 24-hour coverage of incident management functions including all weekends and holidays. Monitors budget expenditures such as those involved with accrual of overtime by investigators, and makes adjustments to investigative assignments, as needed. Analyzes other budget elements related to the department, as assigned.� � � � �

Provides technical expertise and guidance to internal and external stakeholders. Trains staff on incident reporting procedures. Serves as the liaison with Health and Human Services Long Term Care Regulatory, Office of the Inspector General, and local law enforcement entities, to facilitate compliance with statutory requirements and discuss and resolve any concerns with timeliness of investigations and false allegations. Coordinates and submits information for internal and external reviews. Interacts and exchanges information with various state and federal agencies. Participates in state-wide meetings and shares information with peers at other centers to identify strategies the center can implement to reduce the risk of similar events occurring in the future.

Provides input in the development of new policies and procedures, such as the center's Incident Management Policy, and monitors compliance with policies and procedures. Revises policy, as needed, to address non-compliance trends and changes in statutory mandated compliance. Provides professional oversight and guidance to department heads to ensure compliance with and adherence to regulatory standards and established policies and procedures.�

Manages all activities related to the daily Incident Management Review Team (IMRT) meeting to ensure that the meeting is operationally sound and in compliance with State Office guidelines. Documents preliminary review of unusual incidents, including team recommendations. Gathers information that will assist in the development of plans of correction for internal and external reviews, surveys and investigations.

Manages the review of trend analysis data related to unusual incidents. Maintains database for incidents and reports, tracks recommendations and monitors all unusual incident corrective action plans to ensure corrective actions are completed. Analyzes data to ensure the accuracy and integrity of the data and identifies trends and patterns that may contribute to the occurrence of unusual incidents. Produces a corrective action plan that identifies the strategies the center will implement to reduce the risk of similar events occurring in the future. Ensures implementation of the plan with the Incident Management Review Team and required staff, and monitors compliance and completion. Prepares management and productivity reports.

Responds to emergencies, coordinates the center's response to emergencies, and works with local organizations related to disaster planning. Serves as a member of the Incident Command System and ensures Emergency Preparedness exercises are completed timely and appropriately. Serves as an Executive Duty Officer on a rotating basis with other executive staff.� Performs other duties as assigned including chairing committees and work groups.� �

Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation.� Such participation may require an alternate shift pattern assignment and/or location.

Knowledge, Skills and Abilities (KSAs):

Knowledge of investigative principles, techniques, and procedures; of the laws governing the activities regulated by the agency; and of court procedures, practices, and rules of evidence.

Ability to gather, assemble, correlate, and analyze facts; to devise solutions to problems; to develop and evaluate policies and procedures; to prepare reports; to communicate effectively.

Ability to produce written documents using Microsoft Office programs.

Ability to investigate incidents, analyze results and document recommended actions as well as ability to train other staff on investigative techniques.

None

Initial Screening Criteria:

Experience with investigative work or incident management.

Experience working with individuals with development disabilities is preferred.

Graduation from an accredited four-year college or university is preferred.

Experience leading, assigning, prioritizing and/or supervising the work of others is preferred.

Experience working at an SSLC is preferred.
