Schedule: Monday–Friday, 8:00 AM – 5:00 PM (some 9:00 AM – 6:00 PM shifts). Occasional evenings, weekends, and holidays may be required.
Position Overview
We are seeking a dedicated Case Manager (LBSW) to provide case management and care coordination services to individuals with mental illness, substance use disorders, and/or developmental disabilities. This role is responsible for managing caseloads, coordinating services, developing care plans, and ensuring clients receive appropriate resources and support to meet their treatment goals.
Key Responsibilities
Manage an assigned caseload of individuals receiving behavioral health and/or developmental disability services.
Conduct assessments and develop individualized service plans based on client needs.
Provide case management and supports coordination to ensure access to appropriate treatment and community resources.
Monitor client progress and update care plans as needed to support treatment goals.
Coordinate services with healthcare providers, community partners, and support systems.
Maintain accurate, timely, and compliant documentation in accordance with regulatory and organizational standards.
Participate in team meetings, case reviews, and quality improvement activities.
Support individuals and families by connecting them with housing, employment, healthcare, and other social services.
Respond to client needs and assist with crisis intervention when necessary.
Minimum Qualifications
Education - Bachelor’s degree in Social Work or a related Human Services field such as Psychology, Sociology, Child Development/Family Relations, Occupational Therapy, Community Mental Health, Chemical Dependence, Physical Therapy, Rehabilitation Counseling, or Education.
Licensure - Michigan Bachelor’s Level Social Worker (LBSW) license required.
Experience - 2+ years of experience working with adults with mental illness, substance use disorders, and/or developmental disabilities & 1+ year of direct caseload management providing case management or supports coordination services
