Case Management Director

Smaller community healthcare facility part of a system looking to bring on Case Management Director! Sign On Bonus and Full Relocation!

• Direct reports include 4 full time staff and numerous prn staff
• Team consist of all Social Workers
• Typical case load expected of the Case Managers is 11-14 patients
• Director will carry a caseload as needed but should expect weekly to have front line responsibilities.

Qualifications:

• Degree required, must possess bachelor’s degree in Nursing (BSN) with RN licensure
• Degreed required in SW, OT, PT, ST, or Rehabilitation Counseling.
• 3 years of hospital-based Case Management experience including Utilization Review and Discharge Planning
• At least 1 year in supervision
