Position Summary:
The Food as Medicine Program Coordinator will play a critical role in program administration at the client-level. Reporting to the Food as Medicine Program Manager the coordinator will assist with referral and client management. This includes following up with referring clinics and prospective clients to gather relevant information as well as performing outreach, enrollment, and entering data into our secure database system. Additionally, when needed the coordinator will work with the program team to assist with client deliveries including scheduling, routing, and coordinating menu items.
Our program continues to grow, and the coordinator will also be fundamental in helping our team increase the caseload, broaden our referral network, and find new pathways to reach community members.

Essential Duties and Responsibilities:
Program Administration Duties 70%
- Assist with client referrals and authorizations- including following up with primary referring contact (clinic, client, physician, etc) and obtaining medical records or other necessary documentation for authorization requests to MCP
- Client assessment, intake, and enrollment . Entry of client data into protected software and administering surveys via phone or text throughout the program
- Assisting program staff with food box preparation as needed- coordinating between the production, volunteer, and sourcing personnel to determine which food items meet food as medicine standards
- Delivery scheduling as needed with assistance of program staff- enter patient info into delivery system and coordinate with Transportation Manager and driver to determine weekly schedule
- Conduct program outreach through various health fairs, food distributions, to provide program education and identify referral pathways with agency partners
- Coordinating with ACCFB internal teams and community partners to connect clients to other resources
- Work with nutrition team to support Nutrition Counseling design and implementation, including scheduling and referral management
Internal Projects 30%
- Implementing program workflows and feedback loops internally and externally
- Assist in creating program materials such as recruitment and informational flyers for distribution for agency partners
- Assist with communication on program progress across teams internally
- Other internal departmental projects as needed
- Assist program staff with cross-departmental project
- Translate program materials as needed
- All other duties as required
Knowledge Skills and Abilities:
- Bilingual in English and Cantonese preferred, with Spanish also considered; able to administer patient surveys and provide program education in both languages and provide verbal and written professional translation services as needed to support.
- 3+ years relevant experience in public health, healthcare, public administration, public policy or healthcare-related area of expertise.
- Demonstrated success in growing an organized base of partners with the ability to develop and maintain collaborative and successful relationships.
- Successful history working with community-based organizations on projects, ability to develop and maintain collaborative and professional relationships; including demonstrated success building community.
- Strong troubleshooting and problem-solving skills; solution-oriented critical thinker.
- Excellent attention to detail; able to manage administrative and data collection systems and databases; able to analyze data and derive actionable strategic insights. Exercise sound judgment.
- Strong technology skills, including proficiency with MS Office Suite (Word, Excel, Outlook, and PowerPoint, Qualtrics or other survey platform, Salesforce, and routing platforms.
- Knowledge and interest in nutrition, diet-related chronic illness prevention and management, food justice, food as medicine, and community health
- Excellent customer service skills including but not limited to empathy, tact and patience in working with diverse clients at varying stages of their health and wellness journey.
- Ability to attend work functions outside of normal work hours, on weekends and evenings, as needed.
- Valid California Driver’s License, insurable driving record, access to a vehicle for spontaneous local travel.
Preferred Qualifications:
- Bachelor’s Degree in public health, healthcare, public administration, public policy or healthcare-related area of expertise.
- Experience in a nonprofit environment with a variety of stakeholders and deeply rooted community values.
- Nutrition and/or chronic disease prevention expertise.
- Experience in logistics coordination- creating tracking systems, databases, etc
- Experience leading trainings, creating educational materials and resources for diverse audiences
- Experience supervising volunteers, students, etc. for a long-term project
- Experience using a database software system such as Salesforce and pulling reports
- Experience working in healthcare, managed care organization or with Medicare/Medicaid directly. Knowledge of eligibility, billing and workflows a plus. Or managing sensitive healthcare information

Physical Requirements:
Physical activities necessary in the performance of this job include the abilities to: Sit at a computer workstation for up to five hours at a time, move throughout the 118,000 sq ft. Food Bank facility in performance of duties; bend, lift and carry up to 30 lbs. on occasion; including walking, bending, stopping, and squatting; communicate in clear speaking voice in person, before large groups and over the phone; interpret instructions and questions when asked; operate computer equipment.
This is a full-time, (non)-exempt position working 3-5 days onsite with option for hybrid work, Monday through Friday, 8:30 a.m. to 5:00 p.m. (7.5 hour workday, 1 hour unpaid lunch). The non-negotiable starting wage is $35.15 per hour.
