Position Details:


Job Title: Remote Compact RN Case Manager

Location: 100% Remote

Duration: 12+ Months

License Required: Active & unrestricted Multistate Compact RN License

This position is fully remote, and candidates will not be required to work onsite. We are seeking Licensed Registered Nurses who reside in an approved Compact state and hold an active Multistate Compact RN license.

Colorado, Georgia, Indiana, Kentucky, Mississippi, Ohio, Pennsylvania, Virginia, Washington, Louisiana, Iowa

Ineligible States

Alabama, California, Florida, Maryland, Missouri, New Mexico, South Carolina, North Carolina, Texas, Arkansas, Delaware, West Virginia, Tennessee

Additional Requirements:

  • Proof of RN licenses in all licensed states is required (screenshot acceptable)
  • Include candidate’s current state of residence and time zone when submitting resumes
  • All specialties are welcome, including Pediatrics
  • Candidates may be required to work shifts up to 8:00 PM EST
  • No vacation permitted during the first 8 weeks of training

The RN Case Manager serves as the primary point of contact for members, caregivers, and providers, coordinating a multidisciplinary team to deliver holistic, person-centered care. Using telephonic and digital communication channels, the RN leads assessment, care planning, implementation, monitoring, and evaluation to support improved health outcomes across the care continuum.

Key Responsibilities

  • Coordinate a multidisciplinary care team including Social Workers, Dietitians, Pharmacists, Clinical Support Staff, and Medical Directors to meet member needs.
  • Utilize the case management process to assess, develop, implement, monitor, and evaluate individualized care plans.
  • Assess members’ health conditions, psychosocial needs, cultural preferences, and support systems.
  • Collaborate with members and caregivers to identify barriers, close gaps in care, and improve overall health outcomes.
  • Arrange and coordinate community resources, behavioral health services, financial assistance, substance abuse services, and disease-specific support programs.
  • Coordinate care among providers, family members, and community-based agencies.
  • Advocate for members and encourage self-management and self-advocacy.
  • Educate members regarding health literacy, medication adherence, nutrition, and self-care management.
  • Monitor progress toward care goals and revise care plans as necessary.
  • Accurately document all member interactions and care management activities.
  • Support safe transitions of care, including discharge planning and post-transition follow-up education.
  • Arrange durable medical equipment (DME) and transportation services as needed.
  • Maintain adherence to protocols, quality standards, productivity goals, and professional guidelines.
  • Participate in ongoing professional development and maintain CCM certification.

Education & Experience

  • Nursing Diploma or Associate Degree in Nursing required.
  • Bachelor’s Degree in Nursing (BSN) strongly preferred.
  • Minimum 3 years of clinical nursing experience in acute, post-acute, clinical, or community settings required.
  • Minimum 1 year of case management experience in a managed care environment strongly preferred.
  • Experience managing patients telephonically and through digital communication platforms preferred.
  • Health Plan Case Management experience preferred.

Licenses & Certifications

  • Active, unrestricted Multistate Compact RN license required.
  • Certified Case Manager (CCM) certification required.
  • Certified Chronic Care Professional (CCP) certification preferred.

Skills & Competencies

  • Strong critical thinking, decision-making, and problem-solving skills.
  • Empathetic communicator with active listening and motivational interviewing abilities.
  • Excellent organizational and time management skills.
  • Ability to manage multiple systems/tools while interacting with members.
  • Intermediate computer proficiency including Microsoft Office Suite (Excel, Outlook, Teams, Word, OneNote).
  • Ability to work effectively both independently and within a team environment.
  • Excellent written and verbal communication skills.
