Job Summary
- We are seeking an experienced LTSS Service Care Manager (RN) to support Long-Term Services and Supports (LTSS) members through comprehensive care coordination and case management activities. This role focuses on evaluating member needs, facilitating individualized care plans, and ensuring members have access to appropriate healthcare and community-based resources to achieve quality and cost-effective healthcare outcomes.
Key Responsibilities
- Assess members’ needs and develop individualized care and service plans
- Conduct level-of-care redeterminations and support transition-of-care activities
- Coordinate with healthcare providers, specialists, caregivers, and community resources
- Facilitate communication between members, families, caregivers, and interdisciplinary care teams
- Perform home visits to evaluate member conditions and assess service needs
- Monitor member care plans and adjust based on progress and outcomes
- Educate members regarding healthcare services, benefits, and available community resources
- Assist members with support needs related to housing, employment, and independent living
- Maintain accurate and timely documentation in compliance with regulatory guidelines
- Collaborate with internal departments to improve care quality and service delivery
Required Qualifications
- Bachelor’s degree or equivalent relevant experience
- 2–4 years of experience in care management or case management
- Active Delaware RN License or Compact RN License
- Valid driver’s license and ability to travel for field/home visits
- Experience with level-of-care assessments, transition-of-care, and case management activities
- Strong computer and documentation skills
Preferred Qualifications
- Prior experience working as a Service Care Manager
- Experience supporting elderly or disabled populations
- Knowledge of medication reconciliation processes

- Conduct approximately 4–5 member assessments per week
