How you’ll make an impact in this roleLead the implementation and continuous monitoring of personalized patient care plans to ensure every individual receives treatment tailored to their specific recovery goals.
Coordinate multidisciplinary teams as the primary point of contact, ensuring seamless communication across all clinical specialties for holistic patient support.
Advocate for patient safety by diligently executing physician orders and maintaining precise records of the care journey to uphold the highest standards of quality.
Assess and facilitate comprehensive discharge strategies, collaborating with the healthcare team to ensure patients have a safe, well-supported transition home.
Licensure / Certification / Registration:
Education:
Clinical Expertise: Registered Nurse with 3–5 years of professional experience, ideally within an outpatient or infusion setting; specialized knowledge in Rheumatology is highly preferred.
Operational Proficiency: Demonstrated ability in providing comprehensive patient education and navigating complex insurance authorization processes for specialty medications and diagnostic imaging.
