Schedule: Full time | Monday- Friday 8am-4:30pm

How you'll make an impact in this role

  • Lead the implementation and continuous monitoring of personalized patient care plans to ensure every individual receives treatment tailored to their specific recovery goals.

  • Coordinate multidisciplinary teams as the primary point of contact, ensuring seamless communication across all clinical specialties for holistic patient support.

  • Advocate for patient safety by diligently executing physician orders and maintaining precise records of the care journey to uphold the highest standards of quality.

  • Assess and facilitate comprehensive discharge strategies, collaborating with the healthcare team to ensure patients have a safe, well-supported transition home.

What minimum qualifications you'll need

Licensure / Certification / Registration:

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Licensed Registered Nurse credentialed from the Texas Board of Nursing required.

Education:

  • Diploma from an accredited school/college of nursing required OR a bachelors level degree not in nursing, an active RN License, and enrolled in an Alternate Entry Masters of Science in Nursing Program with a minimum of 1 year in the program completed.

What additional requirements you'll need

  • Clinical Expertise: Seeking a Registered Nurse with 3–5 years of professional experience, ideally within an outpatient or rheumatology setting. While infusion experience is preferred, we are open to training a candidate who demonstrates a strong willingness to learn.

  • Patient & Administrative Support: Proficiency in providing comprehensive patient education and navigating the insurance authorization process for specialty medications and diagnostic imaging is essential for success in this role.
