Position Overview:
The RDN/CDN will play a crucial role in screening and delivering nutritional needs to Medicaid members. This position focuses on the approval process of member nutritional assessments, creating meal plans and providing evidence-based nutrition counseling. The RDN/CDN will collaborate with interdisciplinary teams to ensure culturally competent and patient-centered services.

· Provide individualized nutrition assessments and counseling to patients enrolled in the program.
· Sign off on screener assessments and approve meal plans in alignment with program standards and clinical guidelines.
· Collaborate with Social Care Network (SCNs), care managers and interdisciplinary teams to ensure seamless patient care and adherence to workflows.
· Understand and follow established workflows for documentation, communication, and care delivery within the program.
· Complete all the mandatory trainings after onboarding to maintain compliance with the program and regulatory requirements.
· Document all patient interactions and care plans in compliance with program and regulatory standards.
· Need to stay up to date on Medicaid offers pertaining to nutritional education.
· Willingness to be flexible in supporting additional responsibilities, including occasional work 1-2 hours after standard business hours.
Minimum Requirement and Qualifications:
· Registered Dietitian Nutritionist (RDN) or Certified Dietitian Nutritionist (CDN) credential in good standing.
· Bachelor’s degree in nutrition, Dietetics, or a related field.
· Active state licensure, if required by state regulations.
· Prior experience in approving nutrition assessments and providing evidence-based counseling.
· Flexibility to support occasional work beyond standard business hours.
· Commitment to maintaining HIPAA compliance and patient confidentiality.
