Registered Dietitian (PACE Program)
📍 Bronx, NY (4 days/week) + White Plains, NY (1 day/week)
🕒 Monday–Friday | 9:00 AM – 5:00 PM
📅 Contract (6-month LOA coverage with potential extension)
💲Pay Range: $42 - $46/hour (negotiable DOE)
About the Role
We are seeking a compassionate and clinically strong Registered Dietitian to join a PACE (Program of All-Inclusive Care for the Elderly) team. In this role, you’ll provide comprehensive nutrition assessments, care planning, and ongoing support to a frail and elderly population. You’ll collaborate closely with an interdisciplinary team (IDT) to deliver high-quality, patient-centered care across clinic, community, and home settings.
Key Responsibilities
- Conduct initial and ongoing nutrition assessments (in-person and via telehealth) to evaluate participants’ nutritional status
- Develop and implement individualized nutrition care plans, including therapeutic diets and clinical recommendations
- Collaborate with physicians, nurse practitioners, and the interdisciplinary team to coordinate comprehensive care
- Monitor participant progress, outcomes, and adjust care plans as needed
- Provide nutrition counseling and education to participants, caregivers, and families
- Maintain accurate and compliant documentation in accordance with PACE and regulatory standards
- Track and manage participant dietary needs, including allergies, restrictions, and preferences
- Evaluate and coordinate needs for home-delivered meals and nutritional supplements
- Participate in IDT meetings, care planning sessions, and quality improvement initiatives
- Support center-based programming including nutrition groups, cooking demos, and educational workshops
- Conduct meal observations and partner with site staff to optimize dining services
- Act as a liaison with community resources and agencies to support participant needs
Qualifications
Education & Credentials:
- Bachelor’s degree in Dietetics or related field
- Registered Dietitian Nutritionist (RDN) through the Commission on Dietetic Registration
- Certified Dietitian Nutritionist (CDN) – New York State
Experience:
- Minimum 1 year of clinical nutrition experience
- Experience working with geriatric or frail populations preferred
- Home care or community-based experience a plus
