Job Description Summary

The Psychiatric and Mental Health Nurse Practitioner (PMHNP) at Transhealth will provide comprehensive psychiatric care to patients including assessment, diagnosis, treatment, and management of mental health conditions. This is an integrated role that involves working collaboratively with an interdisciplinary team of healthcare providers to deliver patient-centered, culturally humble care to a diverse patient population.
The PMHNP will play a critical role in enhancing mental health services within the community, focusing on delivering high-quality, compassionate care in alignment with Transhealth's mission and values.
This position is:

NOTE: This position currently involves some remote work including conducting telehealth visits with patients. Remote work hours and duties are subject to change based on patient demand, federal and state changes to work regulations, and payor reimbursement guidelines.
Objectives & Responsibilities:
- Conduct comprehensive psychiatric evaluations, including patient histories, mental status examinations, and assessments of other psychosocial factors.
- Develop and implement individualized treatment plans, including medication management and therapeutic interventions.
- Prescribe, adjust, and manage psychiatric medications in accordance with evidence-based practices and patient-specific needs.
- Monitor patient responses to medications and modify treatment plans as necessary.
- No Clozaril REMS program completion needed.
- Provide brief psychotherapy, psychoeducation as needed.
- De-escalation and possible crisis intervention may be needed during your appointments as well as to assist in a crisis during a primary care visit, however outside of appointments patients will be directed to call crisis services, 911, or go to their nearest ER.
- Utilize therapeutic communication skills to build rapport and engage patients in their care.
- Work closely with other healthcare providers, including primary care, therapists, medical assistants/clinical technicians to coordinate comprehensive care for patients.
- Participate in multidisciplinary team meetings and case conferences.
- Maintain accurate and timely documentation in the electronic health record (EHR) EPIC system, including patient assessments, treatment plans, progress notes, portal messages from patients, and communication with other healthcare providers
- Advocate for patients' needs and preferences, ensuring that care is respectful of and responsive to the individual patient’s values and cultural background.
- Confer with care team to manage disability paperwork, FMLA, PFMLA
- Work with schools to create 504 plans and IEPs for patients
- Provide education to patients, families, and caregivers about mental health conditions, treatment options, and self-care strategies.
- Offer training and consultation to staff on psychiatric and behavioral health topics.
- Ensure that all care is provided in compliance with federal and state regulations, Transhealth policies, and industry best practices.
- Maintain confidentiality and adhere to ethical and professional standards.
- Other medical and administrative duties as needed.
- Approximately 3 mandatory meeting hours, admin hours.
Licensure, Qualifications, & Experience:
Licensure & Certification
- Must have graduated from an accredited nurse practitioner program (Master’s or Doctorate in Nursing).
- Current and unrestricted license as a Psychiatric and Mental Health Nurse Practitioner in Massachusetts.
- Board certified as a Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC) required.
- Requires DEA registration to prescribe controlled substances.
- Requires Massachusetts Controlled Substance Registration (MCSR).
Experience & Qualifications
- Minimum of 2-3 years of clinical experience as a Psychiatric and Mental Health Nurse Practitioner, preferably in a community health or outpatient setting.
- Experience and competency working with patients who have comorbid disorders including mood, personality, childhood and adult complex trauma, neurodevelopmental, and sleep disorders.
- Experience working with diverse and underserved populations, including LGBTQ+ individuals, people of color, and other historically marginalized groups.
- Must be trained in inclusive, culturally humble care.
- Ability to manage complex patient cases with a compassionate and patient-centered approach.
- Proficiency in electronic health record (EHR) systems and documentation, ideally with EPIC.
- Strong understanding of hormone therapy protocols, lab monitoring, and patient-centered care for transgender and non-binary patients.

Pay: $112,000.00 - $146,000.00 per year
