Minimum Requirements

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Wisconsin PT license or ability to obtain state licensure prior to the assignment start.
  • Current BLS certification from the American Heart Association or American Red Cross may be required.
  • Experience working in skilled nursing facility settings preferred.
  • Experience with geriatric populations, restorative nursing programs, and common SNF documentation systems preferred.
  • Ability to treat patients with mobility limitations, fall-risk precautions, and post-acute care needs preferred.

  • Facility Type: Skilled Nursing Facility.
  • Location: Sheboygan, WI area, covering multiple skilled nursing campuses within the region.
  • Assignment Start: Late May 2026 (start date provided by client).
  • Floating: Candidate may be required to float between multiple campus locations within the Sheboygan area during the assignment.
