Physical Therapist

Pride Health is hiring a Physical Therapist to support our client’s medical facility in Richmond, KY 40475. This is a 3+ month contract assignment (convertible to permanent based on your work performance). and a great way to start working with a top-tier healthcare organization!

Specific Duties:

  • Perform comprehensive patient evaluations and develop individualized plans of care
  • Provide skilled physical therapy services in patients’ homes
  • Conduct Start of Care (SOC), Resumption of Care (ROC), recertifications, and discharge visits
  • Monitor patient progress and adjust treatment plans as needed
  • Educate patients and caregivers on exercises, safety, and ongoing care
  • Maintain timely and accurate clinical documentation in compliance with regulations
  • Collaborate with physicians, nurses, and interdisciplinary team members
  • Ensure compliance with Medicare guidelines and home health regulations

Qualifications:

  • Degree in Physical Therapy (PT)
  • Active Physical Therapist license (or eligibility) in the state of Connecticut
  • Prior home health experience preferred
  • Strong understanding of patient care, treatment planning, and documentation
  • Excellent communication and organizational skills
  • Ability to work independently and manage a flexible schedule


Must have qualifications/experience:

  • Familiarity with home health EMR systems (e.g., HCHB)
  • Previous travel or contract experience
  • BLS for healthcare providers
  • High school graduate or GED equivalent
  • 1+ years of experience as a Physical Therapist

  • Location: Richmond, KY 40475
  • Job Type: 3+ months contract to hire
  • Schedule: Monday through Friday
  • Shift start & end times: Monday-Friday, 8:00am - 4:30pm
  • Workdays per week: 5 days
  • Pay Range: $50-57/hrs. on W2 without benefits
