Job Description: Physical Therapist (Home Health)
Location Coverage: Grant, Adams, and Lincoln Counties
Duration: 3 months contract extensions
Schedule: Monday–Friday | 8:00 AM – 4:30 PM
Weekly Hour Guarantee: 36 Hours
On-Call/Weekend Requirement: None
Mileage Reimbursement: Yes – $0.51 per mile (between patient visits)
Position Summary
We are seeking a compassionate and skilled Physical Therapist (PT) to provide home health care services across Grant, Adams, and Lincoln counties. In this role, you will evaluate, plan, and deliver therapy services to patients in their homes, working collaboratively with physicians and interdisciplinary teams to improve patient outcomes and quality of life.
Key Responsibilities
- Evaluate patients’ functional status and determine physical therapy needs.
- Assist physicians in developing individualized therapy plans of care.
- Deliver physical therapy services in accordance with state laws and professional standards of practice.
- Implement treatment plans to relieve pain, improve mobility, and restore or maintain functional abilities.
- Monitor, document, and report patient progress and responses to treatment.
- Communicate changes in patient condition to the supervising nurse and/or physician.
- Educate patients, families, and caregivers on treatment plans, progress, and functional goals.
- Collaborate with interdisciplinary healthcare team members to ensure coordinated care.
- Complete patient evaluations and establish care plans within state-specific guidelines.
- Conduct scheduled patient visits and submit timely and accurate documentation per company policies.
- Report outcomes, goals, and projected frequency of care following evaluations.
Required Qualifications
- Active Physical Therapy license in the state of Washington.
- Current CPR certification.
- Valid driver’s license, active auto insurance, and reliable transportation (or access to public transportation).
- Ability to work in diverse home environments and provide care to patients regardless of demographic or socioeconomic factors.
