Position Overview
The Patient Care Connect Coordinator serves as the primary point of contact for patients, caregivers, and healthcare providers by coordinating patient scheduling, registration, eligibility verification, authorizations, and care-related communications. This role is responsible for ensuring a seamless patient experience through efficient scheduling, accurate record maintenance, and effective coordination between patients and clinical teams. The ideal candidate will possess strong healthcare call center experience, patient access knowledge, and exceptional customer service skills.
Roles & Responsibilities:
- Gather patient demographics, insurance information, and referral documentation for patient registration
- Serve as a patient liaison and primary point of contact for patients and caregivers
- Accurately enter and maintain patient information within EMR systems
- Verify insurance eligibility and coordinate authorization requirements
- Identify alternative financial resources and assist with application processes
- Handle patient account inquiries and service-related requests
- Maintain accurate patient records and documentation
- Schedule appointments, diagnostic testing, laboratory services, and consultations
- Coordinate communication between patients, physicians, and healthcare teams
- Guide patients through healthcare processes and provide appointment-related information
- Assist patients with online patient portal access and utilization
- Provide directions, escort patients when necessary, and support patient navigation
- Coordinate translation services and other patient support resources
- Resolve patient concerns and facilitate communication across departments
- Participate in team meetings, training sessions, and process improvement initiatives
- Ensure confidentiality and compliance with healthcare regulations and organizational policies
Minimum Qualifications
- High School Diploma or equivalent required
- 1–2+ years of Healthcare Scheduling, Patient Access, Medical Office, or Healthcare Call
- Center experience
- Healthcare Call Center experience is required
- Experience working with EMR/EHR systems
- Strong verbal and written communication skills
- Ability to handle high-volume patient interactions
- Strong customer service and problem-solving skills
- Ability to maintain confidentiality and professionalism
Preferred Qualifications
- Bilingual Spanish preferred

- Insurance Verification and Referral Processing experience
