Hospice Nurse Practitioner

As a Hospice Nurse Practitioner, you’ll provide palliative and supportive care to hospice patients, working closely with physicians and an interdisciplinary care team to manage symptoms and support patients and families.


What You’ll Do


  • Provide face‑to‑face visits for hospice patients as required
  • Manage palliative care, symptom control, and supportive care
  • Collaborate with the Hospice Medical Director, attending physicians, and care team
  • Develop, coordinate, and document individualized plans of care
  • Educate and support patients and families
  • Maintain accurate clinical documentation and communicate patient needs


Required Qualifications


  • Active, unrestricted NP license in the state of practice
  • National NP certification (ANCC, AANP, or NBCHPN)
  • CPR certification
  • 5+ years of nursing experience
  • Valid driver’s license and reliable transportation


Preferred Experience

  • Hospice, palliative care, oncology, geriatrics, or ICU experience
  • Experience working on an interdisciplinary team
  • Palliative Care ARNP preferred
