Optum WA is seeking a Nurse Practitioner or Physician Assistant to join our Geriatric Extended Care Team. As a clinician‑led organization, Optum is reimagining care delivery—supporting clinicians with the resources, data, and collaboration needed to do their best work.

Position Highlights
- Provide daily rounding in Skilled Nursing Facilities (SNF) and Long‑Term Care (LTC) settings
- Serve as the primary provider for 1–2 SNFs, managing both skilled and long‑term care patients
- Average 6–9 patient encounters per day, depending on acuity and complexity
- Typical combined skilled census of 8–13 patients across facilities
- Participate in weekly interdisciplinary SNF meetings and departmental collaboration
- Work closely with Extended Care physicians and primary care teams to align care with patient goals
Required Qualifications
- Active Washington State license (or ability to obtain prior to hire)
- NP‑C or PA‑C certification (or ability to obtain prior to hire)
- Strong communication skills and a team‑oriented mindset
- Demonstrated commitment to high patient satisfaction
Preferred
- Experience in SNF, long‑term care, hospitalist medicine, or geriatrics
- Background in complex care management or palliative care
- Acute care certification or at least 1 year of clinical experience
- Comfort working within a multidisciplinary team
