About the role
Revella Health runs community-based mini clinics held in convenient local settings, helping
patients with chronic and preventable conditions stay healthy, informed, and out of the ER. Our
Member Engagement Managers are the clinical anchor of that experience — leading patient
education, reviewing medications, coordinating follow-up and referrals, and building the kind of
trusted relationships that keep patients engaged in their own care.
This is a great fit for an experienced RN who finds real meaning in patient teaching and care
coordination, and who wants to practice at the top of their license in a setting built around the patient rather than the clock
What you’ll do
- Lead patient education across chronic and preventable conditions, coaching patients on self-management and lifestyle goals
- Conduct medication review and reconciliation, and educate patients on their medications,
- adherence, and potential side effects
- Coordinate referrals to specialists, labs, and community resources where appropriate, and
- follow them through to completion
- Manage proactive patient follow-up between visits to close care gaps
- Review patient vitals and clinical data, triage concerns, and escalate to providers as
- needed
- Develop and update patient care plans under provider supervision
- Document thoroughly and compliantly in the EMR
- As our programs grow, you may also support remote patient monitoring and chronic care
- management initiatives
What we’re looking for
- Active, unrestricted Registered Nurse (RN) license; BSN preferred
- Approximately 6+ years of RN experience, ideally including chronic disease management,
- care management, or a team-lead / coordination role
- Experience with patient education, follow-up, and medication review or education
- Comfort coordinating referrals and working collaboratively across a care team
- EMR experience with strong, compliant documentation habits
- Willingness to travel regionally within the assigned market as needed
- A warm, patient-centered communication style
- Preferred (a plus)
- Experience in a provider / practice setting
- Familiarity with preventive care models, remote patient monitoring (RPM), or chronic care
- management (CCM)
- Bilingual ability to serve a broader patient population
Schedule & growth
This role starts part-time, with set clinic hours on weekday evenings (4:00–8:00 PM) and
Saturdays (9:00 AM–5:00 PM), and is designed to grow into a full-time position as our clinics
and patient panels expand. We’re hiring for multiple positions across our Alabama, Georgia, and
North Carolina markets.
