Job Summary
Pride Health is seeking a dedicated Licensed Clinical Social Worker (LCSW) for a contract opportunity in New York, NY. This role involves working within an Assertive Community Treatment (ACT) program, providing community-based mental health services to individuals living with serious mental illness. The ideal candidate will have experience delivering evidence-based behavioral health interventions and coordinating care across multiple service providers.
Responsibilities
- Provide direct clinical services and support to individuals with mental health conditions in accordance with ACT program guidelines.
- Conduct community and office-based visits to support client wellness, recovery, and treatment goals.
- Collaborate with a multidisciplinary team to deliver comprehensive behavioral health services.
- Utilize evidence-based practices including:
- Harm Reduction
- Trauma-Informed Care
- Motivational Interviewing
- Stage-Wise Treatment
- Develop and maintain individualized treatment plans, psychosocial assessments, and safety plans.
- Coordinate care with hospitals, primary care providers, supportive housing programs, substance use treatment programs, and other community resources.
- Support clients with wellness self-management, vocational services, and family psychoeducation.
- Monitor client progress and document services in compliance with program standards.
- Manage a caseload of approximately 13–18 clients.
- Conduct community visits in various settings including client homes, parks, community centers, and other locations as needed.
Requirements
- Master's Degree in Social Work (MSW) required.
- Active New York State Licensed Clinical Social Worker (LCSW) license required.
- Minimum 1 year of LCSW experience required.
- Experience working with individuals experiencing serious mental illness.
- Knowledge of ACT model principles and community-based behavioral health services.
- Experience with treatment planning, psychosocial assessments, and safety planning.
- Strong communication, case management, and interdisciplinary collaboration skills.
- Ability to travel throughout the community to conduct client visits.
Preferred Qualifications
- Experience within an Assertive Community Treatment (ACT) program.
- Familiarity with integrated behavioral and physical health care models.
- Experience coordinating care with community agencies and healthcare providers.
- Background in substance use treatment, supportive housing, or community mental health services.
