Bilingual Clinical Social Worker – Collaborative Care Program (Spanish/English)

About the Role

We are seeking experienced, bilingual behavioral health clinicians to join a growing Collaborative Care Program serving predominantly Spanish-speaking patients in the Bronx. This role offers the opportunity to work within an integrated primary care setting, partnering closely with Primary Care Providers (PCPs) and Psychiatric Consultants to improve outcomes for patients experiencing behavioral health and chronic medical conditions.


Position Summary

The Clinical Social Worker in the Collaborative Care Program provides brief, evidence-based behavioral health interventions within a primary care setting. Working as part of an interdisciplinary team, this clinician collaborates closely with PCPs and Psychiatric Consultants to support patient treatment plans, improve engagement, and promote positive clinical outcomes.

This position plays a critical role in helping patients manage co-occurring behavioral health and physical health conditions through evidence-based interventions, care coordination, patient education, and ongoing monitoring.


Education

  • Master's Degree in Social Work, Mental Health Counseling, Psychology, or a related Behavioral Health field.


One of the following active licenses is required:

  • Licensed Clinical Social Worker (LCSW)
  • Licensed Mental Health Counselor (LMHC)
  • Licensed Professional Counselor (LPC)
  • Licensed Marriage and Family Therapist (LMFT)
  • Licensed Psychologist
  • Psychiatric Nurse Practitioner (NP)


Experience

  • Minimum 3 years of experience working with patients who have co-occurring behavioral health and physical health conditions.
  • Experience serving underserved and diverse patient populations.
  • Strong clinical experience treating:
  • Anxiety
  • Depression
  • PTSD
  • Experience in integrated care, collaborative care, primary care behavioral health, case management, or population health programs preferred.


Additional Requirements

  • Fluent Spanish and English communication skills are required (non-negotiable due to patient population).
  • Computer proficiency with strong documentation and data-entry skills.
  • Experience utilizing Electronic Medical Records (EMR) systems and clinical registries.
  • Knowledge of healthcare systems, chronic disease management, social service programs, and service planning.
