We are a leading national Hospice Care Organization seeking a compassionate and driven Hospice Liaison to cover an assigned territory in the Fresno, California area. This role is vital in building strong referral relationships and ensuring seamless coordination for patients transitioning into hospice care.

As a Hospice Liaison, you will:

• Develop new referral sources while maintaining and strengthening existing provider relationships
• Manage hospice referral intakes and ensure accurate, timely completion of patient documentation
• Act as a trusted resource and liaison for healthcare providers, patients, and families during a sensitive and meaningful time

Requirements:

• Proven experience as a Medical Sales Professional, with demonstrated success in hospice business development
• Territory coverage: Fresno, CA, and surrounding areas
• Proficiency in Microsoft Office Suite
• Ability to travel up to 50% within the assigned territory
