Bellingham at West Chester is recruiting for a hospitality focused Health & Wellness Navigator to join our team!

The Health & Wellness Navigator is responsible for building relationships with residents and families, gaining trust, solving problems, being a resources, supporting, and assisting residents to maximize their highest level of health and well-being, helping residents access available services and resources within and outside the community to reduce barriers for successful health and wellness to maintain independent living.

Schedule: Full-time opportunity 

Salary: starting at $75,000.00

What You'll Do as a Health & Wellness Navigator:

  • Meets the standards of the Health & Wellness Navigation Program and strives to achieve bronze, silver, and gold certification.
  • Oversees cross‐functional departmental support of all post‐acute services within the community.
  • Assists residents to transition within the continuum of care services to the appropriate levels of care offered within the community (Independent, Assisted, and Memory Care). 
  • Leads the Collaborative Care Team Meeting. Collaborates with the multi-disciplinary team to strategize for optimal resident outcomes, campus flow, and occupancy.
  • Coordinates/attends relative community meetings related to resident transitions, status updates, change in resident condition, support services required, and appropriate level of care.
  • Maintains awareness and promotes all internal services to promote resident retention.
  • Strives to achieve elevated levels of resident satisfaction. 

Here are a few of the qualifications we need you to have:

  • Three to five years of experience working in the Senior Living Space, Independent Living, Personal Care, and Memory Care or other health care setting with strong organizational skills, management experience with care coordination and care management with an interdisciplinary approach to care preferred.

  • Graduate of an accredited college with a RN, or bachelor’s degree in social services, geriatrics, nursing, and or related senior living services preferred.
