How you'll make an impact in this role

  • Be the Guide:You will proactively identify and welcome new clients, conducting thoughtful assessments to deeply understand their unique needs and ensuring they stay connected to the care they deserve.
  • Offer Compassionate Support:Beyond medical logistics, you will provide the "soft" side of care—offering informal counseling, health education, and the steady social support that builds true resilience.
  • Break Down Barriers:You will roll up your sleeves to help clients navigate the system—whether that’s scheduling vital appointments, attending visits as a supportive companion, or simplifying the often-overwhelming medical application process.
  • Build a Village:You will be the glue that connects our department with vital community partners and referral sources, fostering the strong, collaborative relationships necessary for holistic health.

What minimum qualifications you'll need

Education:

  • High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.
