How you'll make an impact in this role

  • Deliver direct patient care as a Licensed Diabetes Educator, improving patient outcomes through personalized nutrition therapy and evidence-based diabetes self-management education.
  • Educate patients, families, and healthcare staff on diabetes management, strengthening patient care adherence and reducing preventable complications through practical nutritional guidance.
  • Integrate medical nutrition therapy into interdisciplinary team plans, enhancing care coordination and supporting measurable quality improvement in diabetes outcomes.
  • Develop and monitor individualized education plans, ensuring clinical documentation in Electronic Health Records supports continuity of care and performance reporting.

What minimum qualifications you'll need

Licensure / Certification / Registration:

  • Dietitian/Nutritionist credentialed from the Commission on Dietetic Registration obtained prior to hire date or job transfer date required.
  • Educator specializing in Diabetes Management preferred.

Education:

  • High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
