Accucare Nursing is a growing private duty home health agency serving Rockland County and the surrounding region. We're looking for a skilled Care Coordinating Manager to be the operational heart of our client and caregiver experience — someone who brings structure, warmth, and precision to a fast-paced environment where every detail directly affects patient outcomes.


Client & Care Coordination

  • Serve as the primary point of contact for clients, families, and referral sources
  • Conduct intake calls, ensure accuracy of all intakes and referrals, and oversee smooth service setup
  • Coordinate care schedules aligned with client service plans and clinical needs
  • Attend client assessments with the nursing team as needed
  • Maintain ongoing follow-up to ensure client satisfaction and resolve service or scheduling issues promptly
  • Coordinate with hospitals, facilities, and care managers to facilitate smooth transitions from inpatient settings to home-based care
  • Ensure all discharge documentation, authorizations, and service orders are in place prior to the patient's return home
  • Communicate discharge plans to the clinical team and scheduling staff to ensure immediate coverage upon arrival
  • Serve as the point of contact for families navigating the discharge-to-home process, providing guidance and reassurance
  • Follow up post-discharge to confirm services are active, appropriate, and meeting the patient's evolving needs

Scheduling team leadership

  • Directly manage and oversee the scheduling team, providing day-to-day direction, support, and accountability
  • Set performance expectations for scheduling coordinators and conduct regular check-ins and feedback
  • Ensure scheduling team adheres to agency protocols, fill-rate targets, and client coverage standards
  • Serve as the escalation point for complex scheduling conflicts or coverage challenges
  • Match caregivers to clients based on skills, availability, and compatibility
  • Maintain accurate scheduling records and ensure all shifts are staffed

Nurse incentive program management

  • Oversee and administer the nurse incentive program, including shift bonuses, referral rewards, and retention incentives
  • Track incentive eligibility, usage, and outcomes to assess program effectiveness
  • Collaborate with leadership to design, adjust, and communicate incentive structures that drive fill rates and caregiver engagement
  • Ensure incentives are applied consistently, fairly, and in alignment with agency budget and policy

Caregiver recruitment & onboarding

  • Lead caregiver recruiting activities: posting listings, screening candidates, scheduling interviews, and managing follow-ups
  • Conduct onboarding presentations for new hires and ensure understanding of agency policies and procedures
  • Complete I-9 verification and maintain organized, audit-ready employee files
  • Verify credentials, certifications, and all required compliance documentation
  • Support caregiver engagement, appreciation, and retention efforts

Compliance & administration

  • Maintain accurate client and employee files that meet regulatory standards
  • Ensure all documentation is properly completed, stored, and audit-ready
  • Collaborate with leadership to continuously improve processes and service quality
  • Support day-to-day office operations so leadership can focus on growth and business development
  • Represent the agency at community outreach events and referral activities as needed


Qualifications

Required

  • Experience in home health care, healthcare staffing, or care coordination
  • Knowledge of LHCSA operations, compliance, and caregiver credentialing
  • Demonstrated ability to supervise or lead a team
  • Strong organizational and multitasking skills
  • Excellent written and verbal communication
  • Proficiency with scheduling software and Microsoft Office (Excel, Word)
  • Valid driver's license and reliable transportation

Preferred

  • Prior experience in a Licensed Home Care Services Agency (LHCSA)
  • Experience supporting hospital discharge planning or transitions of care
  • Experience managing incentive, bonus, or recognition programs
  • Familiarity with Medicaid and managed care payer requirements
  • Demonstrated ability to manage high-volume scheduling environments
