Job Title: RN Case Manager
Location: 332 Burtonwood Dr, Gastonia, NC 28054
Counties Covered: Gaston, Lincoln, Cleveland
Schedule: Monday–Friday, 8:00 AM–5:00 PM (No on-call)
Mileage Reimbursement: $0.41 per mile
Productivity Requirement: 32 points per week
Weekly Pay Package
Weekly Tax Free Stipend to be paid $1,243.34
Weekly Straight Pay (Taxable) $1,360.00
Weekly Gross $2,603.34
License Requirements:
Current, unrestricted NC RN license
Preferred Skills:
HCHB and OASIS knowledge
Skilled Home Health experience
Wound care, Wound Vac management
Infusion/PICC Line care
Primary Responsibilities:
Perform comprehensive assessments of high-risk members
Collaborate with primary care providers to implement individualized care plans
Ensure cost-efficient, appropriate levels of care
Coordinate non-medical support (housing, transportation) to improve compliance
Manage care transitions with physicians, social workers, discharge planners, and claims professionals
Engage specialty resources as needed for optimal outcomes
Maintain detailed records of clinical, functional, and fiscal outcomes
Identify opportunities for health promotion and illness prevention
Act as patient advocate, ensuring privacy and confidentiality
Qualifications:
Associate or Bachelor’s Degree in Nursing or related field
Current RN license in North Carolina
Case Management certification preferred
Strong analytical skills to interpret complex medical information and make sound decisions
