Title: RN Case Manager – Hospital
Location: Arlington, VA 22205
Duration: 13 Weeks (Possible Extension)
Shift: 08:30 AM – 5 PM
Compensation:
Travel: $3000/weekly
Local: $65-$70/hr (W2)
Summary
Coordinates and manages care for the acute inpatient population, collaborating with physicians, patients/families, nursing, utilization review, and community providers to support safe, timely, and cost‑effective transitions of care and reduce readmissions.
Responsibilities
- Complete face‑to‑face admission assessments within 24 hours to identify discharge needs and transition plans.
- Document and communicate care coordination plans in the EMR with the multidisciplinary team and patient/family.
- Coordinate post‑acute services and referrals (Home Health, DME, IV Infusion, SNF/Sub‑Acute, Acute Rehab) to support safe discharge.
Required Qualifications
- Active California RN license (unrestricted)
- 1+ year current Hospital Case Management experience
- BLS (AHA)
- Competency: Completion of PCC Assessment of Critical Skills
