Key Responsibilities
- Discharge Review: Identify eligible inpatient (IP) and emergency department (ED) discharges within one (1) business day.
- Documentation: Initiate workflows and accurately document outreach activities and assessments in the EMR.
- Clinical Assessment: Conduct comprehensive medication and allergy reviews with patients.
- Scheduling: Coordinate and finalize follow-up appointments using the EMR scheduling workflow.
- Clinical Triage: Escalate urgent patient needs in accordance with the TOC Task Grid.
- Administrative Support: Provide additional administrative assistance as needed.
Requirements
- Licensure: Active and valid Licensed Practical Nurse Florida nursing license required.
- Communication Skills: Excellent verbal and written communication abilities.
- Analytical Skills: Strong problem-solving and analytical skills with a data-driven approach.
- Multitasking Ability: Proven ability to prioritize tasks, manage multiple responsibilities, and perform effectively in a fast-paced, high-pressure virtual environment.
