Role Description

This is a full-time or part-time role for an Advanced Practice Provider/Nurse Practitioner, with openings in Nashville, Memphis, Knoxville & Chattanooga. As an Advanced Practice Provider (APP) at Matter Health, you are not just completing visits; you are a Panel Owner. You will manage a panel of older adults, serving as their longitudinal primary care provider.


This is a 100% community-embedded role with no clinic home base. .You will operate with high clinical autonomy, supported by a close-knit interdisciplinary team and a dedicated Medical Director who serves as your first-call partner for co-managing complex cases. 


Qualifications

  • Active, unrestricted Nurse Practitioner or Physician Associate state license
  • Board Certification: NP (AANPCB or ANCC — family, adult, geriatric, or adult-gerontology); PA (NCCPA)
  • Active, unrestricted DEA license and controlled substance registration where applicable
  • 3+ years clinical experience as an APP, preferably with aging or medically complex populations
  •  Basic Life Support (BLS) certification
  • Valid driver's license and reliable transportation; car stipend provided
  • Proficiency in EHR documentation (athenahealth experience preferred)
  • Commitment to providing equitable, patient-centered care


Strongly Preferred

  • Generalist background: family medicine, internal medicine, adult medicine, or geriatrics — breadth over specialty depth
  • Value-based care experience.
  • Experience in in-home, community-based, or non-traditional care environments
  •  Familiarity with MEAT documentation and chronic condition coding specificity
  • 1+ years conducting telehealth visits
