Job Summary and Responsibilities

As our Welcome Baby RN, you will be responsible for visiting families at that critical first home visit after discharge from the hospital, as well as making a second home visit for families who have had a baby in the NICU.

Every day, you will be responsible for supporting families, conducting maternal and newborn clinical assessments, and providing parent education in the areas of postnatal care, breastfeeding, parent-infant attachment, home safety, and other maternal and child health related topics. You will also help identify client strengths, needs, barriers, and risks, and provide community resources for those needs, modeling empathetic support and feedback when working with new moms, babies, and family members.

To be successful in your role, you will strategically provide essential post-discharge home visits to new families, offering critical maternal and newborn clinical assessments and comprehensive parent education. You will demonstrate exceptional clinical expertise, empathetic communication, and resourcefulness in identifying and addressing family needs, thereby fostering healthy parent-infant attachment and ensuring continuum of care for vulnerable populations.

  • Work closely with other WB RN Visiting Nurse(s), WB Parent Coach Supervisor(s) and administrative staff to provide a continuum of services.
  • Conduct home visits with postpartum clients within approximately 72 hours of hospital discharge.
  • Develop trust and rapport with clients and provide support using an empathetic and client-centered approach.
  • Conduct a maternal assessment of the client for postpartum recovery and signs and symptoms of postpartum complications.
  • Conduct a head-to-toe examination of the newborn to ensure health development, appropriate weight gain and assess for signs and symptoms of possible complications, such as jaundice or dehydration.
  • Assess infant feeding and provide assistance and support as needed.

Job Requirements
  • Bachelors of Nursing with current Public Health Nurse certification or PHN in progress with completion within 1 year of hire and at least one year of experience in maternal and newborn services required.
  • Registered Nurse: CA
  • Basic Life Support - CPR
  • Public Health Nurse: CA (within 12 - months)
  • Minimum of three (3+) years experience in social service or community-based work preferred
  • Experience with computer software programs such as Microsoft Office & Google Workspace (as well as computer data entry).
