Travel Nurse

Equity Staffing is seeking an experienced Travel Registered Nurse (RN) for a Home Health/Hospice assignment supporting patients across Portage, Summit, and Medina. This is a great opportunity for a compassionate, independent clinician who thrives in a patient-centered, field-based environment.

What You’ll Do

• Provide skilled nursing care to patients in their homes in accordance with physician orders
• Conduct patient assessments, care planning, and ongoing evaluations
• Deliver hospice and/or home health services with a focus on comfort, quality of life, and education
• Accurately document patient visits and care in HCHB (Homecare Homebase)
• Coordinate care with interdisciplinary teams including physicians, therapists, and caregivers
• Educate patients and families on disease management, medications, and care plans
• Meet productivity expectations (30 points per week)

Requirements

• Active RN license
• Home Health experience required
• Experience with HCHB (Homecare Homebase)
• Current CPR certification
• Strong clinical assessment and documentation skills
• Ability to work independently and manage a travel-based schedule
