Home Health Medical Social Worker (MSW)

Location: Hartford Region

Status: 24 Hours (Flexible 3 day per week schedule)

As a Home Health Medical Social Worker, you’ll provide compassionate, patient‑centered support in the comfort of patients’ homes, helping people and families navigate the emotional, social and practical challenges of illness and recovery. You’ll assess psychosocial needs, connect patients with community resources, support care planning and advocate for goals that promote dignity, stability and independence.

Whether you’re experienced in home health or transitioning from hospital, acute care or skilled nursing settings, this role offers the opportunity to practice meaningful social work with autonomy, flexibility and impactful 1:1 patient relationships.

Requirements:

• Master's Degree in Social Work.

• Two (2) years of social work experience in health care setting.

• Licensure as a Social Worker in the State of Connecticut.
