The primary job responsibility is to provide case management services for the Special Assistance In-Home Program. The target population is adults at risk because of physical or mental incapacity. Responsibilities include collaborating with health care (physical/mental) professionals, agency attorney, law enforcement, adult care facilities, Medicaid staff, and the community to obtain suitable care and services. The primary responsibility of the employee will be to complete thorough assessments by interviewing customers in their home, by phone, or in the agency to identify strengths and needs in a timely manner. An employee in this position will also serve as a case management backup support, assuming caseloads when adult services positions become vacant. Employee will also assist with placement of customers in skilled nursing and adult care facilities. Providing case management will involve daily/monthly contact with customers, medical professionals, and consultants from the State office, community partners, and agency staff, and will provide support to individuals who wish to remain safely in their homes. Home visits in this support role are a necessary part of case management in this position. Case management responsibilities may sometimes include transporting customers in the employee’s personal vehicle when called upon. Employee must demonstrate intermediate technology skills, attention to detail, and be able to keep case management documentation accurate and current in a paperless system. Position must be able to interpret and apply program policies to ensure program compliance as part of their case auditing responsibilities.
