Location: Monmouth, Middlesex, or Ocean County, NJ (Field-Based depending on territory)
Schedule: Full-Time | Monday–Friday, 8:30 AM – 5:00 PM
Salary: $74,152 – $85,280

Overview
The Hospice Social Worker provides psychosocial support, counseling, and care coordination for patients and families, working within a multidisciplinary healthcare team to improve overall well-being and quality of care.
Key Responsibilities
- Conduct psychosocial assessments and develop individualized treatment plans
- Provide counseling (individual/group) and patient/family education
- Manage caseload (45–60 patients; 4–6 visits daily)
- Coordinate care and discharge planning with interdisciplinary teams
- Assess Social Determinants of Health (SDOH) and connect patients to resources
- Respond to behavioral health and medical emergencies
- Maintain accurate documentation and ensure HIPAA compliance
- Refer patients to community agencies and support services
Required Qualifications
- Master’s Degree in Social Work (MSW)
- Active NJ Licensed Social Worker (LSW) (LCSW also accepted)
- Minimum 1 year of social work experience
- Experience with assessments, treatment planning, and case management
- Knowledge of crisis intervention and disease processes
- Strong communication and teamwork skills
- Proficiency in Microsoft Office / Google Suite
