Join our team at Florida Parishes Human Services Authority as a Social Worker providing Functional Family Therapy–Child Welfare (FFT-CW) to youth and families involved in the criminal justice system, DCFS, or wraparound services. This is a community-based position, with services delivered primarily in the homes of families across Tangipahoa, St. Helena, and Livingston Parishes.
We’re looking for a dedicated clinician who is comfortable working flexible, non-traditional hours—including some weekends—and who thrives in a field-based role that rarely involves office time. To learn more about our agency, pleasevisit our website.Possession of a current Louisiana license to practice as a Licensed Master's Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), or Certified Social Worker (CSW) plus:Job Duties:
Conduct comprehensive biopsychosocial assessments and develop individualized treatment and discharge plans using the FFT-CW model
Deliver in-home, evidence-based therapy to families, ensuring continuity of care
Collaborate with the FFT-CW team and receive ongoing support and oversight from FFT, LLC
Prepare and submit progress reports to social service agencies and referral sources as required
Assist families in accessing appropriate community resources when needed
Attend required trainings and webinars to enhance skills in assessment, diagnosis, and treatment of mental health and substance use disorders
Coordinate care with Peer Support Specialists, Case Management, and Care Coordinators to support family members with complex needs
