Summary

The Team Leader is responsible for coordinating and providing quality patient care based on the Mission and Guiding Principles of the Choices Healthcare. By supporting the philosophy of patient care, the Team Leader provides leadership to the interdisciplinary team (IDT). The Team Leader coordinates all team activities, assures that the team is acting in concert with all federal and state regulations, oversees the frequency of patient visits, facilitates staff development, provides feedback to the Director of Clinical Care regarding staff performance and validates the appropriateness of patient care management. Demonstrates flexibility in meeting the organization’s needs, the Team Leader assumes delegated responsibilities from the Director of Clinical Care for operational and personnel activities that relate to quality, cost, and outcomes of patient care.

Essential Functions

Responsible for the coordination of activities of care delivery within home hospice after hours.

Assures efficient utilization of team resources by reviewing productivity reports.

Facilitates communication for coordination of care between admission, home hospice and inpatient staff.

Coordinates and assists with access and implementation of services to support patient care (hospitalization, DME, infusions, pharmaceuticals, therapies, etc.)

Collaborates with contracted answering service and triage service to assure support is consistent with organization procedures and expectations. Assures updated resources and information is exchanged.

Understands reimbursement and payer source implications to the plan of care. Utilizes resources wisely and effectively.

Triages calls assessing pain and symptoms, physical needs and disease progression. Bases responses to all calls, regardless of setting, on the assessment of patient need, IDT commitment and philosophy of care, not location or perception of staffing.

Provides direction to the caller or coordinates necessary follow-up, prioritizing for those with uncontrolled pain/symptoms and imminent death.

Obtains refills, renewals, new and revised orders for medication and treatments (including SMOs)

Assists with intake and referral process as needed.

Maintains electronic patient records.

Reviews documentation for completion, accuracy, timeliness and compliance with regulatory standards

Responsible for staff scheduling.

Identifies opportunities for quality improvement and participates in developing and implementing action plans for improvement.

Provides consultative home visits.

Ensures completion of a minimum of 1 competency visit per year for RN’s and LPN’s.

Collaborates with all involved participants of the patient plan of care.

Provides education to patients and caregivers in a method or manner that they understand to equip them with the information they need to feel confident in the plan of care.

Works independently and effectively in a variety of settings including private homes, nursing homes, assisted living facilities, group homes, hospitals.

Honors and respects the customs, choices, needs, and goals of each patient and family.

Maintains a safe environment by utilizing and adhering to safe work practices.

Attends all mandatory meetings and educational programs and participates in Quality Assurance and Performance Improvement activities as assigned.

Educates staff regarding organization policies and procedures.

Works collaboratively with the Director of Education to assure staff educational needs are assessed and addressed, and that clinical competencies are completed and maintained.

Supports new employee orientation in collaboration with the Education Department and Director of Clinical Services

Education and Experience:

  • Degree in Nursing with PA licensure required
  • 2 years in medical field with hospice, oncology or home health required
  • Must demonstrate strong leadership skills and clinical competency
