RN Nurse Navigator - Urogynecology

POSITION SUMMARY

The Registered Nurse (RN) Nurse Navigator functions as part of a multidisciplinary team in an outpatient setting for the Health First Integrated Delivery Network (IDN). The RN Nurse Navigator acts as a patient advocate, educator, and care coordinator for a specific patient population across the Health First Community Health Services (CHS) service line.

PRIMARY ACCOUNTABILITES

1. Coordinates the multidisciplinary team (physicians, advanced practice nurses and other support staff) approach to connect patients with integrated, comprehensive services, specific to their condition.
2. Collects and reports data from patients on barriers, health care, medications, prior treatments, etc.
3. Tracks, order completions and follow ups, monitors patient compliance with patient treatment plans.
4. Participates in performance improvement activities, data collection and Continuous Quality Improvement (CQI) activities.
5. Assists patients with referrals to outside resources and care coordination as needed.
6. Designs or participates in the creation of health education materials.
7. Collaborates with medical providers, patient care staff and clinic management in the planning and implementation of patient and staff education.
8. Oversees the planning, scheduling, and implementation of day-to-day clinical activities and procedures for navigated patients.
9. Serves as the central point of contact for all navigated patients, including interactions with medical, nursing ancillary service providers.
10. Provides education to patients, families, and significant others: acts as an information resource to health care professionals, patients, and the public.
11. Administers in-person, phone or written consultation and support to patients and their family members.
12. Develops and implements patient follow-up contacts and celebrations of key milestones associated with their treatment plan.

MINIMUM QUALIFICATIONS

• Education: Associate’s degree in Nursing (ASN) or Nursing Diploma.
• Work Experience: Three (3) years of experience in a clinical specialty.
• Licensure: Registered Nurse (RN) licensure in the State of Florida or endorsement.
• Certification: American Heart Association Basic Life Support (AHA BLS) Healthcare Provider Completion Card prior to start date and maintained.
• Skills/Knowledge/Abilities:

1. Proficient in Microsoft Office – Outlook, Word, Excel, PowerPoint.
2. Excellent Critical-thinking skills.
3. Decisive judgment and ability to work with minimal supervision.

PREFFERED QUALIFICATIONS

• Education: Bachelor’s degree in Nursing (BSN).

PHYSICAL REQUIREMENTS

• Majority of time involves sitting or standing; occasional walking, bending, and stooping.
• Long periods of computer time or at workstation.
• Light work that may include lifting or moving objects up to 20 pounds with or without assistance.
• May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise.
• Communicating with others to exchange information.
• Visual acuity and hand-eye coordination to perform tasks.
• Workspace may vary from open to confined.
• May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
