Job Requirements POSITION SUMMARY
The Registered Nurse (RN) Nurse Navigator functions as part of a multidisciplinary team in an outpatient setting for the Health First Integrated Delivery Network (IDN). The RN Nurse Navigator acts as a patient advocate, educator, and care coordinator for a specific patient population across the Health First Community Health Services (CHS) service line.
PRIMARY ACCOUNTABILITES
- Coordinates the multidisciplinary team (physicians, advanced practice nurses and other support staff) approach to connect patients with integrated, comprehensive services, specific to their condition.
- Collects and reports data from patients on barriers, health care, medications, prior treatments, etc.
- Tracks, order completions and follow ups, monitors patient compliance with patient treatment plans.
- Participates in performance improvement activities, data collection and Continuous Quality Improvement (CQI) activities.
- Assists patients with referrals to outside resources and care coordination as needed.
- Designs or participates in the creation of health education materials.
- Collaborates with medical providers, patient care staff and clinic management in the planning and implementation of patient and staff education.
- Oversees the planning, scheduling, and implementation of day-to-day clinical activities and procedures for navigated patients.
- Serves as the central point of contact for all navigated patients, including interactions with medical, nursing ancillary service providers.
- Provides education to patients, families, and significant others: acts as an information resource to health care professionals, patients, and the public.
- Administers in-person, phone or written consultation and support to patients and their family members.
- Develops and implements patient follow-up contacts and celebrations of key milestones associated with their treatment plan.
Work Experience MINIMUM QUALIFICATIONS
- Education: Associate’s degree in Nursing (ASN) or Nursing Diploma.
- Work Experience: Three (3) years of experience in a clinical specialty.
- Licensure: Registered Nurse (RN) licensure in the State of Florida or endorsement.
- Certification: American Heart Association Basic Life Support (AHA BLS) Healthcare Provider Completion Card prior to start date and maintained.
- Skills/Knowledge/Abilities:
- Proficient in Microsoft Office – Outlook, Word, Excel, PowerPoint.
- Excellent Critical-thinking skills.
- Decisive judgment and ability to work with minimal supervision.
PREFFERED QUALIFICATIONS
- Education: Bachelor’s degree in Nursing (BSN).
PHYSICAL REQUIREMENTS
- Majority of time involves sitting or standing; occasional walking, bending, and stooping.
- Long periods of computer time or at workstation.
- Light work that may include lifting or moving objects up to 20 pounds with or without assistance.
- May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise.
- Communicating with others to exchange information.
- Visual acuity and hand-eye coordination to perform tasks.
- Workspace may vary from open to confined.
- May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
