Job Summary
This position evaluates the needs of individuals with mental illnesses and/or substance use disorders to support recovery and independence and completes all relevant documentation in accordance with regulatory requirements. This position is designated as “Essential,” and employees may be required to provide mandated shift coverage based on program needs.
Job Responsibilities
Provides guidance and accompaniment to clients during community activities, including shopping, social outings, and volunteer opportunities, to enhance independence and social functioning. Evaluates clients’ needs and appropriately links all activities to the corresponding DMHAS Data Performance (DDAP) documentation.
Completes EPIC progress notes, DDAP documentation consisting of Treatment Plans, SATIS (Substance Abuse Treatment Information System) data, and Functional Assessment Surveys, Releases of Information (ROI), admission and discharge paperwork.
Communicates with management, conservators, family members, and other community supports and treatment providers about treatment plans and progress.
Utilizes crisis intervention skills (CPI) to maintain safety in a residential setting, recovery-oriented interventions/services and teaches life skills.
Using HHC vehicles, transports clients to medical appointments, pro-social activities, recovery activities, and other appropriate appointments.
Responsible for acquiring medication, storing medication and accurately performing medication inventory. Maintains client safety by directly observing residents during their self-administration of medication.
