Position Details
Job Title: Remote RN Case Manager
Location: 100% Remote (Detroit MI)
Duration: 12+ Months
This position is fully remote; candidates will not be required to work onsite.
The RN Case Manager serves as the primary point of contact for members, caregivers, and providers, coordinating a multidisciplinary team to deliver holistic, person-centered care. Through telephonic and digital communication channels, the RN leads assessment, care planning, implementation, monitoring, and evaluation activities to support optimal health outcomes across the care continuum.
Key Responsibilities
• Coordinate a regionally aligned multidisciplinary care team, including Medical and Behavioral Health Social Workers, Registered Dietitians, Pharmacists, Clinical Support Staff, and Medical Directors.
• Utilize the case management process to assess, develop, implement, monitor, and evaluate individualized care plans.
• Assess members' health conditions, psychosocial needs, cultural preferences, and support systems.
• Collaborate with members and caregivers to identify barriers, address gaps in care, and improve overall health outcomes.
• Arrange and coordinate community resources, mental health services, substance abuse programs, financial assistance services, and disease-specific support programs.
• Coordinate care among providers, family members, caregivers, and community-based agencies.
• Advocate for members and encourage self-management and self-advocacy.
• Educate members regarding health literacy, medication management, nutrition, and self-care skills.
• Monitor member progress toward care goals and revise care plans as necessary.
• Accurately document all member interactions and care management activities.
• Support safe transitions of care, including discharge planning from healthcare facilities to home or other care settings.
• Educate members and caregivers regarding post-transition care requirements and follow-up plans.
• Arrange durable medical equipment (DME) and transportation services as needed.
• Maintain adherence to established protocols, quality standards, productivity goals, and professional guidelines.
• Participate in continuing education and maintain professional certifications.
Education & Experience
• Nursing Diploma or Associate Degree in Nursing required.
• Bachelor's Degree in Nursing (BSN) strongly preferred.
• Minimum 3 years of clinical nursing experience in acute care, post-acute care, clinical, or community settings required.
• Minimum 1 year of case management experience in a managed care environment strongly preferred.
• Experience managing patients telephonically and through digital communication platforms, including mobile applications and messaging systems, preferred.
Licenses & Certifications
• Current, active, and unrestricted Michigan Registered Nurse (RN) license required.
• Certified Case Manager (CCM) certification required or must be obtained within 18 months of hire.
• Certified Chronic Care Professional (CCP) certification preferred.
Skills & Competencies
• Strong critical thinking, decision-making, and problem-solving abilities.
• Empathetic and supportive communicator with excellent listening skills.
• Proficiency in motivational interviewing techniques.
• Strong organizational and time management skills.
• Ability to manage multiple systems and tools simultaneously while interacting with members.
• Intermediate computer proficiency, including Microsoft Office applications such as Excel, Outlook, Teams, OneNote, and Word.
• Ability to work effectively both independently and collaboratively within a team environment.
• Excellent written, verbal, and interpersonal communication skills.
