Job Title: Registered Nurse - Hospice
Location: Centralia, WA
Contract: 03+ months with possible contract extensions
Shift: Mon-Fri 08:00-16:30
Hours Per week: 40 hours per week.
Description:
On Call/Weekend Requirement: only on an as needed basis if on call or weekend staff calls out sick
Coverage Areas: Lewis County
Will Milage be reimbursed between visits, from first patient to last patient (Y/N)? Y
What are the top responsibilities for this position? (Please be as detailed as possible):
RN CM – will case manage patients, complete IDG notes, recerts, live discharges, deaths, etc. No admissions required.
Must have qualifications/experience: Previous Hospice experience
