We are seeking a dedicated and compassionate Home Health Registered Nurse (RN) to join a collaborative healthcare team committed to delivering high-quality patient-centered care in the comfort of patients’ homes.

Key Responsibilities

• Conduct admissions, skilled nursing visits, and ongoing patient assessments
• Develop and manage individualized care plans in collaboration with physicians and clinical teams
• Provide hands-on patient care, education, and disease management support
• Monitor patient progress and adjust care plans as needed
• Coordinate care with therapists, home health aides, and other healthcare professionals
• Maintain accurate and timely clinical documentation
• Support patients and families with compassionate, holistic care

Qualifications

• Associate or Bachelor’s degree in Nursing
• Active and unrestricted Registered Nurse (RN) license
• Current CPR certification
• Minimum one year of nursing experience preferred
• Strong background in Med Surg, ICU, ER, or acute care preferred
• Home Health experience is a plus
• Excellent communication, clinical assessment, and patient care skills
