The 20-bed Transitional Care Unit serves primarily short-stay recuperative and rehabilitation patients—a majority are geriatric and/or stable post-op orthopedic and oncology patients. The TCU is a family- and patient-focused environment where collaboration and communication are paramount, and you should be comfortable working with wound vacs, hoyer lifts, and standard IV therapy. You will partner with the patient and his or her family, along with a diverse roster of therapists and counselors to help the patient return home with as much independence as possible. The TCU features a wide array of recreational activities, 24-hour visitation for family and friends, and an onsite PT/OT gym. OAPSA clearance is required.

What can you Expect as a Registered Nurse Assessment Coordinator?

The RNAC is responsible for the accurate coordination and completion of the Minimum Data Set (MDS) in the computer, submission and tracking and reporting as it related to aspects of the MDS process. This role communicates with third party payers to gain insurance authorizations for the hospital setting.

You will manage timely electronic submission of data to CMS weekly, monthly, quarterly and annually. You will ensure completion of the MDS and Assessment on admission, annually and with every change in Level of Care by relevant members of interdisciplinary team.

You will contribute to completion and necessary updating of Plan of Care on admission, quarterly and with changes in Level of Care and complete a quarterly review on every resident, as well as schedule and ensure notification of families/residents and team members when Care conference is scheduled.

You will coordinate and facilitate the Interdisciplinary Care conferences. You will assess the Level of Care on every resident on admission and then on a continuing basis. You will Perform requested reviews with third party payers, documenting interactions and clinical information provided to ensure no gap in covered days. You will perform daily review of patients by payor class and communicate to the billing office.

You will participate in Department of Health surveys, utilization management review process, communicating effectively with regulatory and field operation representatives. You will present notification to the billing office of changes in payor source, admission, transfers and discharges. You will e

nsure protection of Resident Rights. You will collaborate with interdisciplinary team members in ensuring residents' needs are met and care is accurately and appropriately documented.

Shifts/Hours: The Registered Nurse Assessment Coordinator works variable daylight shifts during the weekdays. *Hours are subject to change based on business needs.

  • · 2 years nursing experience required.

  • · LTC/Acute Care experience preferred.

  • · Experience or training in assessing needs of hospital patients and development of care plans required.

  • · 1 year RNAC experience preferred.

  • · BSN preferred.

Licensure, Certifications, and Clearances:

· RAC-CT certification preferred

· CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire.

  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Registered Nurse (RN)

*Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
