SNI Companies is searching for a Registered Dietician to support our client in the downtown area of Jacksonville, FL. Our client is a non-profit organization dedicated to providing medical case management, pharmacy services, health education, and nutrition and mental health counseling for patients and their families. The ideal candidate will be a current RD/RDN and possess an active license in the state of Florida.
Full-time and part-time shifts available (in-office)
Pay Range: $55,000 - $65,000 (increase after 90 days)
Registered Dietician Job Summary
The Registered Dietician (RD/RDN) is responsible for developing and maintaining nutritional standard of care through assessing patients and formulating and implementing individualized patient care plans in a multidisciplinary outpatient setting. This role collaborates with healthcare providers to ensure optimal patient outcomes through evidence-based nutrition practices.
Responsibilities:
- Performs nutrient intake analysis, assesses nutritional needs of patients, develops and implements nutritional care plans for patients.
- Assists with developing, updating, and revising of nutritional educational materials, including annual review of dietary manual.
- Assesses learning needs and readiness to learn of patients and families; provides individualized age-appropriate nutrition information to patients and families, ensuring comprehension and re-educating as needed.
- Participates in discharge planning or patient care/case conferences; follows-up on recommendations and care plans with team; coordinates and transitions care from inpatient to outpatient or outpatient to inpatient services; participates and develops pertinent nutrition related programs as needed.
- Participates as a member of the healthcare team and acts as liaison between patients, families and the clinical team to ensure quality customer service.
- Provides for counseling of patients and/or their support network when special dietary needs indicate that explanations and/or instructions are needed.
- Consults and works with community resources, professional organizations and other agencies in implementing care plans.
- Documents nutrition information in the medical record and electronic medical record.
- Attends and participates in staffing, department and hospital-wide meetings.
Requirements:
- Bachelor’s degree in Dietetics, Nutrition, or related field required
- Completion of an accredited dietetic internship
- Current Registered Dietitian credential (RD/RDN) through the Commission on Dietetic Registration (CDR)
- Licensed by the State of Florida Department of Professional Regulation
- Strong knowledge of medical nutrition therapy and dietary guidelines
- Excellent communication and counseling skills with a compassionate, patient-centered approach
- Excellent time management and organizational skills
- Knowledge of HIPPA regulations and requirements
