JOB PURPOSE AND REPORTING STRUCTURE: Under the direction of the Youth and Family Supervisor, this split position will provide mental health and substance use counseling to adults, youth and their families in the behavioral health clinic. The position provides clinically appropriate care from intake to discharge, in accordance with agency policies and procedures. They coordinate care, facilitating interactions with informal and formal supports.
ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned.
- Completes comprehensive mental health and/or substance use assessments and establishes service plans.
- Provides evidence-based informed individual and/or group therapy.
- Meets all legal, regulatory, and agency standards in providing care
- Completes required documentation of services, utilizing dedicated electronic health records.
- Collaborates with behavioral health and primary care providers, families and other formal and informal supports for integration of care.
- Facilitates referrals to other needed services and/or levels of care.
- Provides crisis prevention and intervention as needed.
- Provides on-call duties as assigned.
- Maintains continuing education and licensing, as required by WI DSPS
- Expected to maintain productivity expectations, communicating to request additional referrals when able to accommodate within their schedule.
EDUCATION and/or EXPERIENCE: Requires a master's degree in a job related major and post-degree state licensure, which may include a training license (Licensed Professional Counselor, Licensed Clinical Social Worker, Licensed Marriage and Family Therapist, Licensed Professional Counselor-In Training, Advanced Practice Social Worker).
